Our food bank self referral South Tyneside form lets you request an emergency food parcel if you do not have a keyworker. After you submit the form, we will email to confirm whether you can collect. Please wait for confirmation before attending. Bring proof of address to collect.
Please complete the form below to self-refer to our food bank for yourself or someone you know, giving a short explanation of why the food parcel is required. Please be aware you will need to bring proof of address when collecting your food parcel. We kindly ask that members of our Community Shop service do not request food bank parcels as these will NOT be approved. Please speak to your respective Community Shop Manager where you need to raise such a request in confidence.
The Food Bank is open Monday, Tuesday, Thursday and Friday between 10am and 12 Noon. We do not open on a Wednesday. Our Food Parcels are intended for an emergency supply of food, in a crisis situation, and not as a long-term situation. Specific items cannot be guaranteed to be present in the Food Parcel.
In response to these challenging times, we are currently allowing self-referrals to our food bank, for individuals and families in South Tyneside experiencing food poverty. If you find yourself in need of a food parcel due to a change in your circumstances and do not have access to a keyworker through DWP or local agency, please complete the form and advise us of your situation.
We will respond to your request within at least one working day and email you to confirm if you can attend and collect your parcel. Please do not attend until you’re invited to do so. A new referral will need to be completed for future requests. Please look to include an active email address with your referral wherever possible as not doing so can cause a delay in receiving your parcel.
Sadly, we cannot guarantee distribution of any toiletry items and we ask you request (should you need to) no more than three individual items per parcel. This is done to ensure supply for as many people as possible. Thank you.
